Help Me Grow Long Island educates residents across 4 hospital systems: NYU, Northwell, Stony Brook, and NUMC about our services and ways to best connect with families about a developmental concern.
Help Me Grow Long Island Resident Resources
HMGLI Referral Form: Use this form for submitting referrals to HMGLI. Please ensure the parent consents to receiving text messages and phone calls from HMGLI.
NY State Disability Sheet: Children under the age of 3 diagnosed with any of the conditions on this are automatically eligible for guaranteed Early Intervention services without first having to prove a specific developmental milestone delay.
HMGLI Bilingual Learners Sheets: As children learn multiple languages in the home there may be questions as the child develops. This sheet available in English, Spanish and Mandarin addresses that developmental process.
Lantern: This is the texting platform we partner with, free for our families, that offers trusted parenting advice/tips as well as information on local community events/resources based off of a family's zip code.
Ages and Stages Questionnaire: In both Spanish and English (ASQ-3 and ASQ-SE) are offered via our website at every stage of development.
CDC Learn the Signs Act Early: Webpage where parents can find resources on milestone attainment including a CDC app for tracking development. LTSAE outreach materials can be ordered for patients through outreach@hmgli.org.
